Last name first and then abbreviate first and middle names to just the first initial with no spaces, commas, or periods in between. Place a comma between directors. End with a period.

The title & subtitle are separated by a colon. Capitalize all major words of the title and subtitle. Italicize the title.

Put the format such as DVD or videotape in brackets followed by a period.
